High School Team Registration: Consent Forms

In addition to the general team registration requirements, each high school team must complete and submit a set of consent forms. Consent forms are required from all high school track participants including instructors, advisors, students, and others.

A team may submit their consent forms using the online consent form system OR the paper consent forms. You do not need to submit both an electronic consent form and a paper consent form. Please note that the team roster deadline is September 13, and submit your consent forms at least 1 week before so iGEM HQ will have enough time to process them.

  2. Each PI, instructor, and advisor/mentor must submit a participant consent form
  3. A principal of each high school represented on the team must sign the principal consent form

If your team is using laboratory facilities outside of the high school, the team will need to submit the following consent forms:

  1. Each high school student must submit a participant consent form signed by his/her parent or guardian
  2. Each PI, instructor, and advisor/mentor must submit a participant consent form
  3. A principal of each high school represented on the team must sign the principal consent form
  4. The department head in charge of the laboratory facility used by the team must sign the department head consent form

The original hard copy of the consent forms must be sent to iGEM Headquarters at the address below. We strongly recommend shipping the consent forms via a carrier that will provide a tracking number and providing that number to documents AT igem DOT org.
