High School Consent Forms

In addition to the general team registration requirements, each high school team must complete and submit a set of consent forms. Consent forms are required from all high school team participants including instructors, advisors, students, and others. We must receive a consent form for each team member in order to approve his/her iGEM.org user account. In order to approve a team, we must receive the following consent forms:

  • Principal consent form for every school represented on the team
  • Participant consent form from the primary PI of the team
  • Department head consent form (if the team is working at a laboratory outside of their high school)
