What is Human Practices: Our Insight at a Glance
Human practice plays very important role in project. It can provide the way for us to develop 'project which is integrated with all the values of the elements of society'. It provides as all the requirements that the society
set. To do a comprehensive human practice work, we list all the society elements who was possible to give the critics, advices, suggestions and feedbacks--the stakeholders. We found much stakeholders in vary level--from the level
of RT (rukuntetangga; small-scale community leader), religious leader, to the level of province government or university.
Besides, we can 'give back our project' to the society by doing social projects, empowering communities, doing seminar etc. Then, we include all the factor that maybe affect us in working, such as limited facility and
far-located laboratorium (the facility is located in 32 km outside the city), lack of experience due to it is our first participation in iGEM and SynBio competition--we are the first team from Sumatera to join this etc. Along with
HP, we can do so much to our society instead of just doing thing inside the lab.
Developing Framework
For HP, we develop a framework which contains all the elements about the project and the society. The main point is how make the project become the society-integrated project. So, we start from determine the problem,
select the possible stakeholder, develop the programs from the problems and the possible stakeholders, and integrate it to project. The framework is on prism-refracted light shaped, so we called it by Sriwijaya Refracted Light
Framework Model for Human Practice.
Main Problem of The Project
Lung cancer is a significant public health problem in Indonesia. Today, the incidence of lung cancer at all ages in Indonesia is 12.4% of all cancer. Deaths from lung cancer at all ages in Indonesia are 10.9 percent.
The new cancer case growth is also substantial, amounting to 30,023 new cases by 2018 in Indonesia.
Right now, the only way to make the diagnosis is through the biopsy. But biopsy is invasive, need a
expertise to do the procedure, and cannot be done in very sick patient. So, Soseromarker examinations such as CEA have become very promising because of the many weaknesses. The sensitivity of the CEA examination for lung cancer
was 17.15 ± 0.1% and the specificity of the CEA examination for lung cancer was 96.6 ± 0.02% 14. CEA examination combined with other tests will produce a diagnostic effort whose specificity and sensitivity are not much different
from histo-pathological examination.
CEA examination carried out using the principle of enzyme-linked immunosorbent assay (ELISA) can only be done in laboratories that have special reagents that are not
practical enough and are very expensive. For this reason, a change in the CEA detection method as a diagnostic support tool to make it more practical, cheaper and even accessible to people who do not have special abilities in primary
health facilities will be very important.
But soon, we realize that in order to complete the project and know the various demands of the society, we must look at various aspects. Expert in various fields
(not only lab consultants) became important because the proposed project would be implemented not just in the lab, but also in the first health facility, the medical center, and the public. Apart from a lab problem, the legal umbrella
problem was a major problem. The input from the provincial government as partners and regulators has also been crucial.
The team point is important too, since we need to have good chemistry to do our
jobs. It must be that we don’t have that chemistry at the first time because we all from many faculties in different campus place, separated 32 km away. At last, we don’t forget to involve the iGEM itself to our project. Since
we have no experience in the competition, a sharing session with alumnus would be so impactful. Also, we meet the member of iGEM HP Committee itself to make sure that our job is at the right place.

- Lung Specialist Doctor - Mr. Joni
Dr. dr. Joni Anwar, SpP is an experienced lung specialist doctor. After got his doctor title from Sriwijaya University, he continue his study in University of Indonesia.
At first, we don’t know what is the problem of lung cancer diagnose method. After met him, we know that the circulating tumor marker is uprising. The biopsy requires certain facility, ceratin people, and certain patient.
We also know that CEA is widely used for the seromarker of lung cancer. The sensitivity of the CEA examination for lung cancer was 17.15 ± 0.1% and the specificity of the CEA examination for lung cancer was 96.6 ± 0.02% 14. Muhammad Firley Lakoni, A.Md. is an experienced local MC, fashion designer, and former presenter of Go GoGasing! In a national TV channel, Astro TV.
We met him for the tips to make a effective communication to people, educate people, and how to develop a good social media strategy. He suggests us to use well-known term instead of medical term to educate people. And to do that, we better use supporting tools such as poster, leaflet, etc. And because the medical thing is ‘very boring’, he suggests us to attach the education with something that attract people. - Law Expert - Mr. Syahri
Muhammad Syahri Ramadhan, S.H.,M.H. is one of the lecturer in Faculty of Law, Sriwijaya University. He finishes his bachelor in Sriwijaya University and Master in GadjahMada University.
When we struggling with law issue in the genetic experiment, Mr. Syahri came to rescue. He said that there is no specific regulation for genetic experiment. He said that our law only regulates about the medical intervention after the product is ready, regulated by the Act 29/2004about Medical Practice. It stated that one of the doctor duties is increasing his knowledge and following the development of medical science. So, the project is the part of it.
Then he suggested us to patent the project as soon as possible after the project is done. The patent is regulated in Act 13/2016 about Patent, as long as the product is not against
Act 13/2016 clause 9. That clause regulated the invention that cannot be given the patency is: 1) the product is against the law, religion, public order, and decency; 2) the product is against the right science and method. The project must clinically tested and approved by the governmental institution. - Ethical Expert - Mr. Erial
One of the Faculty of Medicine Sriwijaya University Ethical Committee member, dr. Erial Bahar, M.Sc.
We meet the ethical committee to make sure that our project is not harmful. From the meeting, we know that the safety aspect is important, especially in production and application. So, we have to pay attention to:
1. Ensure that the laboratorium is as the standard level.
2. Ensure that the personal protective equipment is as the standard level and worn everytime in the production and application of the product.
3. Ensure that the organism that the team used is not harmful and pathogenic.
4. On the process of production and application, make sure that the procedure is handled by competent worker (for example the laboratorian, the health professional, etc).
5. Ensure that the product is passed the ethical review by the ethical committee. - Chief of Public Health Pakjo, Palembang - Mr. Asnawi
iGEM has conducted an interview with one of the stakeholders who has an authority in Palembang, namely the head of the Puskesmas (public health center). iGem stands for International Genetically Engineering Mechine, with our project circulating around early detection for lung cancer research. As we know that lung cancer is difficult to detect, the time of the advanced stage or the end can only be detected so we are an iGem team from all disciplines ranging from the faculties of law, physical science, medicine, public health, engineering, economics, computer science to natural science collaborating to conduct research on early detection of lung cancer. So from the discipline of public health, the role of finding information about stakeholder views about our project, then get information about lung cancer and ask for opinions and suggestions for us going
forward.
Based on the explanation from the head of the Pakjo Puskesmas for lung cancer cases during the last 2 years there have been no reports, but for lung cancer symptoms such as shortness of breath, pulmonary tuberculosis, cough, and so on already exist. The urgency of lung cancer itself because the biggest risk factor for lung cancer is in cigarettes. As we know the people of Indonesia, especially in South Sumatra in the current season with a lot of cigarette consumption. Moreover, young smokers have experienced a very significant increase. So from that there is a special program to prevent lung cancer, even though there are no rates of lung cancer in the working area of Pakjo Public Health Center. So because the biggest risk factor is smoking, then a program related to lung cancer prevention is appropriate in one of the cities of Palembang to suppress smoking rates with the issuance of regulation number 7 of 2009 on non-smoking areas and in the Pakjo Public Health Center to implement the regulation, the effort to overcome the effects of smoking is to hold a bhaya cigarette counseling for school students starting from elementary school, we are targeting in grade 6 and in junior high school all students, then high school.
This activity is called UBM (Smoking Eradication Efforts) so there is counseling, after counseling the measurement of CO is held in the lungs, after being measured if CO exceeds the normal limit of more than 12 it is recommended that the school counseling the child who has been exposed to the cigarette. Then monitored routinely with counseling teachers to ensure the number of active smokers among teenagers or school children. Another program, which is to supervise, every month oversees 3 areas, namely educational institutions, places of worship, public places. Puskesmas Pakjo came to the place and reminded the management that there was a regulation on KTR in public places and gave a smoking ban sticker in public places and so on. So those two things have been carried out.
The average schools have been entered namely SMP N 22, SMP N 33, MTSN2, MAN 3, SMA N 11, SMP N 25, SMP N 25 and for UBM and KTR Supervision. There are many obstacles in running these two programs, because this regulation has been socialized but the supervision is lacking. So if it is only burdened to the health department and the puskesmas cannot be handled entirely, maybe in the future it will be more effective if all components such as schools and others collaborate in implementing this local regulation. Like schools, public places and others are able to be responsible for their places, such as worship place managers are able to supervise and so on. In the future, such control can walk, because so far it has been running. If the counseling is related to funding and human resources, it is hoped that in the future there will be more people who care, not only the health department and puskesmas but other communities and institutions also care about this local regulation.
For cooperation, Puskesmas Pakjo has partnered with PKK for UBM, and for the community there is not yet, it is hoped that more partnerships such as student communities, ambassadors of the genre and others who care about health especially smokers. So the synergy of other communities to reduce the number of smokers can increase

On 16th June 2019, We met the Head of Rukun Warga that carried out the society in smallesrt community in goverment, Mr. Erwinsyah after scheduling the meeting weeks before. During the interview we explained about our project and how it will contribute to the society, we asked for opinions on how to educate and socialize our project to the public about the dangers of Smoking which could cause lung cancer and we were met with very positive response from Mr. Erwinsyah. He told us that our project really good for the society since based on data the number of lung cancer getting increase. He support us a lot for campaigning lung cancer in his society.
- Dean of Public Health Faculty - Mr. Iwan
On 20th August 2019, We met the Dean of Public Health Faculty, Sriwijaya University, Mr. Iwan Setia Budi S.KM., M.KM. In that Meeting we inttoduce our team, our project and what will we contribute to the public health, we also asked for advices on how to educate the public about the dangers of Smoking which could cause lung cancer and he really interest about our project since lung cancer being so urgent nowadays and he gave us tips on how to educated the society and do a health campaign. He also believe our project could benefit for the public health in the future and he do support us for the project.
- Health Office of South Sumatera
On 20th May 2019, We met the Head of Provincial Department of Public Health, Mrs. Lesty after scheduling the meeting weeks before. Mrs. Lesty also invited several of her staffs and she even called a doctor from the Lung Hospital of South Sumatra to assist us. During the interview we explained about our project and how it will contribute to the public health, we asked for advices on how to educate the public about the dangers of Smoking which could cause lung cancer and we were met with very positive response from Mrs. Lesty and the rest of the
staffs,they educated us on how to do a health campaign, told us about the morbidity of Smokers in South Sumatra and what has the government done to prevent it.
They said they believe our project could benefit for the public health in the future and statedthat they will fully support us and will always stand by if we need any help and even wished for our success for the competition.

This Grand Meeting we held on July, all the member attend this Grand Meeting. We were talking about how our project gonna do and telling about proggress in every division so that each of division could help each other. Another thing is we were talking on how our project will really impactful for society.

This Small Meeting we conduct on September we were talking about our urgention on our project and our deadline. Beside that we analyze how impactful the program and event that we have been conduct so far. After that we evaluate the analysis data that we have got.
- BioSafety Training
This Training we held on May, we were having discussion with Dra. Lesty Nuraini, Apt, M. Kes., Head of Provincial Department of Public Health about Biosafety. She told us how we should safe on doing the research and she fully support this project because it will really beneficial for the

- Synthetic Biology Seminar: ABOUT LUNG CANCER
On 5rd October 2019, we held a Seminar titled 1st Sriwijaya SynBio Seminar: ABOUT LUNG CANCER. This seminar becomes the 1st ever seminar about Synthetic Biology in our university, Universitas Sriwijaya. The seminar was held in Universitas Sriwijaya campus of the Faculty of Medicine located in Palembang. The Speaker was dr. ZiskeMaritska, M.Si., Med whose material was Introduction to Synthetic Biology and dr. Joni Anwar, Sp. P with Diagnosing Lung Cancer via Biomarker. The participants of the seminar was a group of diverse university students, there were medical students, nursing students, Biology students, etc. We are glad to held this seminar, because we can give information about lung cancer to participate and share an information about iGEM too.
- Lung Cancer Demo and iGEM Demo
This campaign was carried out in one of our parks in Palembang, namely the kambang iwak park. Around 50 people of various ages and levels of education who were in the park participated in this campaign. The campaign began by asking about lung cancer and its risk factors to find out how people's knowledge about lung cancer. After that we share information about what lung cancer is, how to reduce the risk factors and how important it is to do early detection if there are symptoms.
Positive response is given from the community. We also share knowledge about the projects we are working on, which are expected to help create an easy, inexpensive and fast diagnostic tool for lung cancer. After this campaign they become more aware of the dangers of lung cancer, how to prevent and the importance of early detection of this disease.
